New graduates may start employment under Graduate Nurse license status (per the State of CT DPH guidelines) with pending license and must successfully complete first NCLEX testing and hold active license within 90 days from graduation date.. The Care Management team supports the department's mission to provide support to patients in maintaining health and wellness in the outpatient setting. The Senior Nurse Case Manager reports to the Manager of Care Management and plays a pivotal role in assessing member needs, providing clinical education, and coordinating care with providers, medical groups, and community resources. Certified Case Manager (CCM) Certification or in the process of completing certification when eligible based on CCM application requirements. A minimum of 3 years of managed care experience in inpatient, outpatient, or managed care environment is preferred. Strong knowledge of healthcare delivery systems, managed care principles, and care coordination.
DEFINITION: Nurse Educators are responsible for communicating class content to students so that learning occurs, skills are developed, and students are motivated to continue to learn and to achieve their educational objectives, in the classroom and in a hybrid distance learning environment.. Nurse Educators must log into and/or engage with their courses and students within the Learning Management System (LMS), five of seven days each week..
