Float RN Case Manager - Discharge Planner.. As a member of a multidisciplinary care management team, the nursing case manager is accountable and responsible for coordinating the care and services of selected patient populations. Works closely with third party payers to resolve problems and issues affecting the continuity of patient care. Coordinates and directs the discharge planning of patients with family members, health care team and the community. Reviews admission data within the designated timeframe to determine the suitability of the level of care in accordance with ISD-A criteria and/or established criteria.

Registered Nurse with Bachelors of Science in nursing (BSN) degree or Registered Nurse with Associates degree in nursing (ADN) plus five years utilization review or case management experience required. Registered Nurse with Masters of Science in nursing (MSN) degree or Registered Nurse with Bachelors in nursing (BSN) plus five years utilization review or case management experience required. Demonstrates skill in educating patient, family and interdisciplinary team regarding post-acute care options, status determination, and other care coordination services. As a day shift, full-time, Care Manager and Discharge Planner, you would be working for the only Level I Trauma hospital within Albuquerque, NM. Conduct initial and ongoing assessments, initiate disease management protocols, determine and manage outcomes, ensure continuity of care through discharge planning, utilization of resources and analysis of variances.. ORDERS AND REFERRALS - Obtain necessary orders from physicians to initiate home health referrals, home infusion medications and supplies, oxygen and equipment; coordinate referrals for oxygen and equipment. ORIENTATION - Participate in orientation, continuing education of staff RN's and other health care team members as appropriate. PATIENT CARE - (UPC and CPC Only) Write treatment plans; coordinate patient drug and procedure activities; administer medication and treatment; provide and coordinate nursing care of assigned patients; may facilitate group therapy and/or education sessions
