Hospital-Based Nursing

Gentiva Hospice is a member of the Gentiva family – an industry leader in hospice, palliative, home health, and personal home care. This position will directly report to the Executive Director RN IPCC and function under the direction, instruction, and supervision of the Registered Nurse. Certified Nursing Assistant (CNA) license. Six (6) months of patient care experience in critical care or acute care (such as ICU, IPU, ER or emergency room) strongly preferred. Additional, preferred experience to help you be successful: hospice, home health, oncology, hospital Staff RN, Med Surg, PACU, Float pool, cardiac, geriatric, admissions, wound care, telemetry, nursing home or travel.

Collects, analyzes and records relevant patient health data accurately, with no more than 3-5 exceptions, as measured by random supervisory chart review and/or direct supervisory observation and co-worker feedback.. Analyzes assessment data to identify patient problems, with no more than 2-3 exceptions, as measured by random supervisory chart review and/or direct supervisory observation.. Implements and records interventions appropriate to diagnosis or patient problem, with no more than 3-5 exceptions, as measured by random supervisory chart review and/or direct supervisory observation.. Adheres to approved, research-based practice standards, with no more than 2-3 exceptions, as measured by direct supervisory observation and random supervisory chart review.. Assures that patients have informed consent signed for indicated procedures, 100% of the time, as measured by random supervisory chart review.

If in a Critical Care area an Advanced Cardiac Life Support (ACLS) through American Heart Association (AHA) Certification must be presented upon hire or within first year of employment.. If in an Adult/Combined EDs, ACLS upon hire or within one year of beginning work in the Emergency Department.. If in an ED, Pediatric Advanced Life Support (PALS) through American Heart Association (AHA) upon hire or within one year of beginning work in the Emergency Department.. Any registered nurse or advanced practice nurse must obtain a cancer specific certification or demonstrate ongoing qualifying education within the timeframe of the facilities accreditation cycle, if they work in medical oncology, radiation oncology, cancer center or cancer clinic and/or administer chemotherapy within an accredited Cleveland Clinic facility.. Minimum one year current acute care RN experience and/or skilled nursing facility (SNF) experience required for SNF care settings.

In this role, you will be responsible for the management and delivery of direct nursing care to PACE (Program of All-inclusive Care for the Elderly) participants in a variety of settings including, but not limited to, the clinic, adult day center, home, virtually, SNF and beyond.. Conducts face-to-face nursing assessments that are inclusive of physical, psychosocial, and behavioral statuses in various settings (home, day center, virtual, long term care facility, etc.). Provides case management during transitions of care and proactively communicates with hospital and long-term care facility case managers for care coordination.. Attributes of an Ideal Candidate Passion and mission orientation for serving high-risk seniors and frail older adults.. Demonstrated experience in wound care, IVs, phlebotomy, colostomy/ileostomy care, etc.

Under general supervision of the Chief Nurse Executive (CNE) and Associate Chief Nurse Executive (ACNE), the Inpatient Unit Nurse Manager supervises delivery and evaluation of nursing care and the care environment in an ambulatory clinic setting.. Provides nursing duties in the care of general medical-surgical, urgent, emergency/trauma, obstetrical, pediatric and behavioral patients encountered on a daily basis.. Determines the current and future direction for the department is based on ongoing evaluation, market analysis, provider practice, reimbursement trends and fiscal viability.. Participates in the overall risk management activities for the Division of Nursing in collaboration with CNE, ACNE and Nurse Educator to assure timely follow up and action plan; with reports sent to Risk Management and Safety Officer..
