Outpatient clinic with a mixture of telehealth and home hea
Key Responsibilities Provide full-scope primary care including preventive services, acute care, and chronic disease management Perform patient evaluations, physical exams, and routine screenings Order and interpret diagnostic tests, labs, and imaging Develop and manage individualized treatment plans
Nurse Practitioner | Family Practice. Monday through Friday 8 am - 4:30 pm
40 hour work week with no overtime
15 patients per day with longer visits
Clinic setting
Geriatric patient population ages 55 and up
PACE program experience preferred
Hospital privileges required
2 years NP experience require
