Family Nurse Practitioner (FNP) or Adult Nurse Practitioner (ANP). Minimum 2 yrs exp as NP preferably with transplant, nephrology, or complex chronic disease populations. Duties will include working in a clinic 3 days a week and 2 days a week doing comprehensive health work via telehealth for lab review and follow-ups. Primary Location: 425 Post Road, Fairfield - Provider will be expected to see patients at multiple sites once training is completed, as we have satellite offices.. This role has the potential to evolve, possibly including rounding at SVMC and assisting providers in the OR. However, the core responsibility will be focused on seeing patients in the outpatient clinic..
